Non-NHS Medical Report Requests Information Sheet

Mairches Medical Practice offers a Private Fee based service for the completion of Medical Reports.
Upon submission your request will be reviewed by our Medical Report Team.
The Medical Report Team will contact you to inform you of the relevant fee which will be applied. In some cases there will not be a fee, the Medical Report Team will advise you appropriately when they contact you.
Once you have confirmed you would like to proceed and you have paid the associated fee the Practice will process your request.
Timeline – your request will take circa 6-8 weeks to complete. Your patience is appreciated and you are welcome to contact the Team for an update at the 6 week point.
Fees are periodically revised and updated.
