Request Acute Medication

Order your ACUTE prescriptions online below. You must be registered with the Practice before we can accept your request.

Please note that there are two forms – one for each of our practice locations, Hawick or Galashiels. Please note that we are only able to complete requests made to the correct practice you are registered with.  

This form should only be used for requesting acute medication.

Repeat prescriptions can be ordered via patient access by clicking here. Any requests received for repeat prescriptions will be rejected.

This service will be on a pilot basis and we will review this in 3 months. Please allow 7 full working days for acute prescriptions to be processed, as per NHS Borders policy. Remember to take weekends and bank holidays into account.

Please note: For reasons of privacy this form will not store your details or medication request. There is no email acknowledgement with this service. Once you send this form a notification message will appear to indicate successful submission. The details you submit will be sent to us by unencrypted email via the Internet. This means that complete confidentiality is not possible because we cannot guarantee the security of the Internet. If you are not comfortable with this arrangement you may prefer to order your prescription in person, or via your Pharmacy.
